- Document Requirements:
- Letter of Intent (LOI)
- Accomplished EAC ETEEAP Application Form
- Accomplished CHED ETEEAP Application Form
- HS Form 137/HS Form 138/ PEPT Certificate/ Certificate of Transfer Credentials/ Official Transcript of Records
- Original Birth Certificate (NSO or PSA Authenticated)
- Certification of Employment (present and previews employment)
- Certifications of Job Descriptions
- Certificates of Seminars attended, certificates of trainings, certificates of merit
- Certificates of Recognition, Certificates of Distinction, Awards, and the like
- If working in Government-Owned, Controlled Corporation (GOCCs), and Private Organizations:
- Industry Certifications
- Has at least five years of relevant work experience.
- Must be at least twenty-five (25) years old; and
- Filipino Citizen
- If Tech-Voc and has worked abroad:
- National Certifications (NCs) from TESDA
- Photographs of the candidate in the workplace or work situation if Overseas Filipino Worker (OFW)
- Passport entries and visas (OFW)
- If with Business Operations:
- Business permit and other business documents
- Licenses and tax payments
- Other documents in support of claimed competencies/experiences
- Other Requirement/s:
- NBI/Police/Barangay Clearance
- Two (2) pcs. latest Picture 2×2 (colored, photo taken on white background)
- Long clear Book (Red cover for BSBA and Blue for BS Criminology)
Step 1. APPLICATION AND EVALUATION
- Submits letter of intent and fill-up EAC ETEEAP Application Form
- Proceed to ETEEAP Dean concerned for an initial interview and evaluation
- Proceed to the Cashier’s Office for payment of application fee.
- Proceed to Guidance Placement Office for an assessment test
- Submits portfolio along with the supporting documents indicated in the List of ETEEAP requirements.
Step 2. PANEL INTERVIEW
- Proceed to the Cashier’s Office for payment of Assessment Fee
- Answer the inquiry of the panel of assessors and perform skills demonstration if needed.
Step 3. WORKPLACE VISIT / TELECONFERENCE
- Proceed to the Cashier’s Office for 1st installment payment
- Assist the needs of faculty assessors for the conduct of workplace visit/teleconferencing
- Follow-up the result of evaluation
Step 4. ADMISSION AND ENROLLMENT
- Encodes personal and educational information in the online admission platform
- Proceed to ETEEAP Office for encoding of courses/subjects
- Proceed to the Cashier’s Office for payment of program miscellaneous fees
- Proceed to Registrar for the issuance of Official Registration
- Proceed to the Office of Student Affairs for ID Application
Step 5. ENRICHMENT / SUPPLIMENTATION
- Attends classes through prescribed learning modality
- Proceed to the Cashier’s Office for payment of required amount for final installment.
- Verifies with the Director the schedule of the research presentation
Step 6. COMPLIANCE WITH GRADUATION REQUIREMENTS
- Fills out and submits the application for graduation to the registrar
- Attends the graduation ceremony and is awarded with the degree
